AFIB Innovation Program
The AFIB Innovation Program is a project directed at redesigning the system and process of care for atrial fibrillation (AFIB) to improve patients’ quality of life and to improve the sustainability of the healthcare system from the Emergency room to the Family Doctor’s office. It is being run by collaborative team of Heart Rhythm Specialists, Neurologists, Internists, Family Doctors, Nurse Practitioners, Pharmacists, Researchers, Designers and Engineers that span across multiple healthcare organizations including the Centre for Innovation in Complex Care at University Health Network, St. Michael’s Hospital and the Taddle Creek Family Health Team.
The AFIB Innovation Program is suprted by the Ontario Ministry of Health and Long Term Care through the Building Bridges to Integrate Care (BRIDGES) initiative and through an unrestricted research grant from Boehringer Ingelheim Canada.
